There are like 15 types of prescribing nurses in the US. Do you have specific issues or questions about them, or did you want to just imply that you think thousands of people are incompetent and move on with your life?

NPs/APRNs definitely have their place. Midwives allow safer births outside the hospital, FNPs can handle most common complaints and perform simple procedures like removing impacted earwax without a physician present, hell even anesthetists have their place. I even heard of one neurosurgeon who hired an NP to do a full neuro exam on all his patients before he saw them in clinic - a huge time saver and beyond what many neurosurgeons do with their ambulatory patients.

Where it gets sketchy is using NPs in place of IMs and even intensivists. Mid level providers get used a lot on stroke floors in my hospital system. You see a LOT of inappropriate discharges, like GCS<7 full codes with 200+ systolic BP being sent to home care.

Not sure if you're the same guy but you've got me really interested in medical imaging. But still my biggest worry is how my Laryngopharyngeal Reflux will affect my work. I think it'll be a problem for all careers, but more so for jobs where I have to talk a lot.

I can project my voice relatively well to begin with but it becomes painful and my projection declines as the day goes on. After 3 years it has improved considerably but my recovery seems to have plateaued since the first year. I might have it for life. To give you an idea, if you and I were having a one-on-one casual conversation, I could last about 1 hour until I become uncomfortable and after 2 hours the pain would be unbearable. So it's important that I'm not talking in a loud environment and also that there's breaks inbetween talking. If I was working with patients, I would probably wouldn't like small-talk or jokes because I'ld want to minimise my vocal output as to conserve my fragile voice.

How many patients do you see a day?
How long are you talking to the patients for?
Are you talking with co-workers constantly?
Are there times when you are filling out forms / configuring settings on equipment where you don't have to talk at all?

My dad had his his aortic valve and part of his aorta infected and got a valve and aortic root replacement. He was in and out of the hospital for months before the surgery and he had a pretty bad prognosis. He went to some big shot cardiothoracic surgeon in San Diego and had got fixed right up. Wasn't sure at all desu. I'm a UK student and I know US med school is very different but here's my view. I only decided to do medicine because it has like 100% job security and I like problem solving. I also didn't want to be a cog in the corporate machine like both my parents. I think I would have been equally suited for any engineering degree as well. In my first two years I questioned my decision all the time but after my third year and now in my fourth year I couldn't imagine doing anything but medicine. I'm aiming for anaesthetics.

All the higher calling caring for people stuff is bullshit in my opinion, if you really only cared about helping people you would be a care assistant or a nurse or something. But you have to be able to turn that side on for all the entrance/interview stuff and OSCEs.

Hi /med/, sorry for being giga gay and marching in here with a question but i've searched for fucking years now and i want an answer, maybe anyone of you will recognize what i'm talking about?

So, basically, whenever i don't sleep much in the night (like, 4 or 5 hours of sleep, not enough), i have this weird thing during the day.

From time to time, i will SUDDENLY remember a dream i had. Or not. What i mean is - i'm not sure if i actually HAD that dream, or my brain is making me think i had. Either way, what happens is i completely lose track of what i was doing and "focus" on the dream that i just remembered - all this while having a slight headache and very slight problems with breathing.

And this lasts 10 seconds, or so - i was never able to count. Either way, once it's done - The headache is gone, i can breathe normally, and more importantly - I completely FORGET the "dream" i just had remembered. Poof. Gone. Not a single detail.

When describing that to a friend of mine he said it might be related to high blood pressure, which i do indeed have - but it seems SO bizzare, especially given i can't find any details on it on google.

Any help? Does anyone know what happens to me?

It’s a specialty where you’ll have to do a lot of reading to stay up to date, and there is a lot of paperwork. Still if you enjoy the problem solving aspect of medicine, treating complicated or multiple diagnoses, managing medications, and long term care then you might find it fulfilling. As a kid I suffered from severe headaches 3-4 times a week. After tons of useless doctor visits I eventually found my way to a major city where I got a consult from a very smart otolaryngologist and got endoscopic sinus surgery along with correction of a deviated septum. Research is not necessary but it’s helpfu, the majority of matriculants have some research experience. It doesn’t even necessarily have to be in biology or chemistry, the point of it is that you know a little about the research process. Volunteer work is required, clinical and non clinical. I’d recommend you look on SDN at the non-traditional forum.

Volunteer, take the pre-reqs (get a 4.0), take the MCAT, and apply early and broadly. Some medical schools are more open to older students than others so researching that will help you.

It often helps to get a good detailed profile of the complaint then progress through the history narrowing it down as you go along. It saves you from spending loads of time working through set questions. The relevance of your questions regarding diet and recent travel are completely dependent on details of the complaint.

Try to lift your head up while using your hands to not allow it to move. Do this for 3 seconds, then relax for 3 seconds. Take a deep breath and pull your head further down with your hands as you exhale. Congratulations, you just performed muscle energy and stretched your neck muscles. You can do this to ANY muscle in your body that you can provide resistance for.

I have a nose surgery scheduled due to a deviated septum, around 90% or so apparently. I went in for congestion, this explains it. I also ask for the surgery, it wasn't forced onto me. Apparently he also saw that the turbinates are enlarged moderately. But that could largely be because allergy season, but I dunno.
I read up on consequences of surgery not going well, and I'm scared. I'm considering asking him to not touch the turbinates, and only straighten the deviated septum to be "safe".

MS2 anon here, so I haven't experienced clincal years myself, but know plenty of MS3/4s and talked with them about it.

A significant amount is school dependent, but generally: First two years is like college on steroids. You'll be putting in 3-4 hours a day minimum of study time, plus lectures. My school also has a decent amount of practical things, like learning how to do history and physicals, practice on standardized patients, etc.

Clinical years are you going to clinic for the majority of your day (though this depends on what service you're on - psych for example usually has much shorter days than surgery), and then studying for your shelf exams. Just how hard you'll have to study depends on your school - some put high emphasis on the shelf-exams as part of your clerkship eval, some care more about what the docs you're working with have to say.

Damn. Well for the past couple days my condition is stable enough - no fever, no rashes, no difficulties in breathing, no hypotension. Rather tame as sicknesses go, but what do I know?
The coughing only comes up in the morning for an hour or so after I wake up (i.e. until enough phlegm is purged out), and starts up nighttime around like 7 PM. All other times in between are alright, not a single cough.
But like I said, I'm gonna wait until this Friday and hope it goes away. If it still persists, THEN it's time to go see a real doctor and not randos on a mongolian basketweaving forum.

Probably less than 15% of practicing DO's uses OMM (osteopathic manipulative medicine), and probably less than half have OMM make up a significant part of their practice.

For the most part, OMM is a low risk way to provide some symptomatic relief. There are some techniques that have been shown to actually help (such as colonic stimulation for post-op ileus). The stuff that's used to treat Musculo Skeletal complaints, like low range of motion due to fascial adhesion, or tight/shortened muscles, tend to work the best for patients. The more voodoo stuff like cranial rhythm, lymphatic flow techniques, and sympathetic/parasympathetic stuff is mostly placebo.

There are also a subsection (generally older DO's) of DO's that go full quackery/scam artist with crystals, funky diets, etc.

I don't think I've ever seen a DO claim this. But, what they could be talking about is relieving some of the symptoms with osteopathic techniques.

Depends on the pathology, if it's a middle ear infection, or middle ear effusion, there are some osteopathic techniques that help open up the Eustachian tube (the tube from the ear to the pharynx), and help drain it out, which would help with hearing.
