I was a volunteer on the phase III trial for the Valneva vaccine against Covid-19 and this is exactly what they did.
The Valneva vaccine was a "traditional" vaccine IE an injection of deactivated / denatured virus, not an mRNA or viral vector.

However, the "control" group on the study received the AstraZeneca vaccine. The rational given was that it would be unethical to give the control group a saline shot and allow them to think, mistakenly, that they had some degree of protection against the virus.

Another trick is exclusion criteria. Candidates in the intervention arm are excluded mid-trial if they develop any of a list of pre-selected (post-selected) criteria.
To make up for it, the same number of random participants in the placebo arm are also excluded.
This was (in)famously used to hide the literal deaths in the first two sets of efficacy trials for Pfizer and AZ covid jabs.
No-one batted an eyelid because according to the rules of "le science" they can exclude any safety signal - Because they never did safety trials. They were only efficacy trials (Efficacy signal being antibody response, not efficacy against infection).

Its different for every trial, but generally yeah, placebo doesn't always mean placebo. A lot of the time it is the drug just abscent the active pharmaceutical ingrediant.

We won't know for the Covid trials within our lifetime. All the trial data is sealed.
